2024 Program Enrollment Agreement (Apply Online)

Complete all required fields (indicated by *), digitally sign and pay the $125.00 registration fee.

NOTE:  If you are under the age of 18, a parent or guardian signature is required on the Agreement.  Please provide a parent email address on the Agreement and it will be sent to them to digitally sign and date through Adobe Sign.

Once you complete the Agreement and pay the registration fee, HIWT will send notification of your start date/shift via email.
